We offer everything from low to no-cost direct services, financial support, and workshops that allow us to deliver on our commitment to help, educate and serve the marginalized and underserved populations in South Florida.
ABOUT OPTIMAL HEALTH FOUNDATION
Welcome to Optimal Health Foundation, a community-based organization with a vision to build a community in South Florida where quality healthcare services are available and affordable for all.

We are dedicated to improving the health and well-being of individuals facing financial challenges, with a particular focus on reducing the underserved gaps that cause health disparities affecting minority communities.
Our mission is to provide the community access to support in the areas of Healthcare Services, Sexual Health, and Mental Health.
We do this through providing supplemental health services, linkage to care, and other resources to support individuals in maintaining their health. Through our various programs and partnerships, we are able to offer a range of services to meet the diverse needs of our clients. These services may include transportation assistance with accessing medical care, financial assistance with prescriptions, health education initiatives and resources, and much more.

SEXUAL Healthcare
Services
At Optimal Health Foundation, we partner with health centers to provide fast, free, and/or low-cost healthcare services, including sexual health services. We understand the importance of sexual health and offer a range of services to support individuals in maintaining their overall health and well-being. Our sexual health services include Sexual Transmitted Diseases (STDs) testing and treatment, birth control counseling, and education on healthy sexual practices. We also offer tele-health services, allowing individuals to access our services from the comfort of their own home. Our tele-health services are convenient, confidential, and secure, making it easier for individuals to receive the care they need.

Our HIV counselors are committed to maintaining confidentiality while providing personalized one-on-one care to clients. They offer education on risk reduction methods for HIV and other sexually transmitted infections (STIs), and work closely with clients to address their unique needs and concerns. Our goal is to provide a supportive and non-judgmental environment where clients can receive the information and care they need to make informed decisions about their sexual health.

Transportation Assistance
We offer transportation assistance to and from medical appointments for members.
Medication & Co-pay Assistance Programs
We offer medication and co-pay financial assistance for members with qualifying prescriptions.
Care Packages

We offer care packages which include essential hygiene items for members of the community in need.
Food Assistance
​
We have food assistance programs which include grocery vouchers and free meals weekly.
Housing Assistance
​
We offer connections to housing assistance programs for members in the community.

We believe that by providing access to primary care and mental health services, we can help individuals achieve optimal health and well-being, and reduce healthcare disparities in our community. To accomplish our goal, we offer a range of services to meet the unique needs of each individual, including medication management and referrals to community parters. Our linkage to care program is designed to be flexible and adaptable to meet the changing needs of our clients. Our organization is committed to providing high-quality, compassionate care to all individuals, regardless of their background or circumstances.
